Young people in vulnerable situations often receive support from multiple services at once, such as education, healthcare, child welfare and welfare services. Today, these services are often poorly coordinated. This can lead to fragmented support, delays in receiving help, and a lack of continuity in follow-up. At the same time, rates of mental health challenges, social exclusion, and dropout from education and work are increasing among young people.
The project will develop and test a new model for more integrated and coordinated follow-up of young people aged 15–19 and their families. The aim is to strengthen collaboration between services, improve information sharing, and involve young people and their families more actively. This will make support more coherent, targeted, and adapted to individual needs.
This is particularly relevant because current service systems are fragmented, with different regulations, responsibilities, and ways of working. The need for better coordination is well documented, but there is limited knowledge of how this can be achieved in practice across sectors.
SINTEF contributes with research and innovation methods throughout the project. We lead the work on user involvement, co-creation, and the development of a new model, and are responsible for piloting and evaluation. Through close collaboration with municipal services, users, and other stakeholders, the project will generate knowledge that can be applied in other municipalities.
